Primary Purpose: The primary purpose of this position is to conduct intakes with clients and coordinate services among regional partners with the goal of ensuring stable housing for clients.
The Homeless Prevention Program (HPP) is a statewide initiative designed to assist households (individuals and families) who are at high-risk of becoming homeless. Essential Functions: Receive referrals for the Fairfield County region from the statewide 211 system Initiate contact with individuals
and families at risk of eviction to begin the application process Provide education to clients and property owners about the HPP Assist clients in completing an intake application form and collect documentation to verify program eligibility.
Utilize the statewide HMIS database to collect data, request financial assistance from program fiduciary, and record client outcomes Complete HUD Habitability Standards in accordance with program guidelines Conduct needs backssment of the household to determine a plan to prevent eviction Coordinate services between program components including mediation services, legal services, and/or case management services Provide ongoing coordination between
HPP service components until the client's housing is stabilized Participate in statewide and regional HPP meetings as needed Qualifications: Bachelor's degree AND two years of experience Bilingual in Spanish, both written and oral required Valid CT Driver's License, proof of insurance and access to reliable transportation Proficient in Microsoft Outlook, Word, Excel and Power Point Preferred experience in housing and housing law Knowledge of leasing procedures and fair housing laws is preferred Knowledge of HUD housing quality standards (HQS) and inspection methods is preferred High level of organizational skills and ability to prioritize and manage multiple projects simultaneously Highly motivated, creative with excellent interpersonal skills Excellent written and verbal communication skills If you are looking for something new and exciting and want to join a fast growing and innovative organization, we'd love to work with you!
