Understand use of quality measures in primary geriatric care and participate in quality improvement initiatives Coordinate care between the New Bridge on the Charles Medical Practice and specialty services at other locations, via tracking of clinical information and facilitating communication between staff members as needed Collaborates with other members of the team (including but not exclusive to the MD, NP, SW, clinic coordinator) to meet the clinical needs of the patient and monitor outcomes of care Provides administrative coverage when needed (e.
g. when the clinic coordinator is not available). Adheres to best practice recommendations and implements evidence-based interventions to ensure optimum outcomes for patients Documents pertinent information regarding care delivered in designated areas of the electronic health records Identifies and seeks out necessary resources as needed to ensure the provision of safe and effective care Assumes accountability for his/her own learning needs and professional development Demonstrates critical thinking abilities in problem resolution Role models professional behavior Actively participates in shared governance, committees, etc.
