Responsibilities also include information assurance, maintaining database policies and procedures, and ensuring data confidentiality, integrity, and availability. Database Development, Management, and Maintenance Maintain the overall integrity and quality of the FDACS databases, including regular data improvement and hygiene, enforcement of database business processes and management of code values, business rules and other logical/physical functions of the databases.
Optimize the use of the databases across the organization by staying current on upcoming enhancements to functionality and guiding staff in the appropriate use of the existing functionality. Provide support and training in the use of the database environment, including orientation for all new staff and specialized training in response to changing roles, business processes and database functionality. Work with technical teams and help desk to provide technical assistance to users and troubleshoot and resolve application and database-related problems. Manage the relationship with software vendors, escalate issues that require immediate resolution, and advocating for fixes/patches to the applications and software when appropriate.

Oversees the design, evaluation, selection, implementation, and support of major databases and metadata structures. Provides extensive technical, and strategic advice and guidance at the highest level to senior managers and technical resources in the creation and implementation of new data architecture, standards, and databases.
Provides a strategic level of technical leadership to the database administration and programming groups, Information Technology teams, and the enterprise. Engage FDACS Business and Technology teams and Business Analytic stakeholders to understand business and operational data needs. Implement and maintain a data warehousing architecture strategy and design an incremental roadmap that overcomes current challenges and appropriately meets current and future analytic needs. Plan and align current and future data warehousing capabilities to support enterprise and business priorities and initiatives and address the resulting capability gaps/needs.
Evaluate backup procedures and ensure that they are performed daily. Make recommendations to management on the selection of database management systems and related software. backss future information requirements to develop long-range, comprehensive database plans. Communicates frequently with Admissions to ensure timely follow-up. When assignment is complete, informs Admissions and prepares for next assignment. Provides all necessary clinical communication timely using SBAR. Discusses any potential needs with after-hours staff. Obtains consents for services and arranges safe discharge in an efficient manner for referred and eligible patients while meeting productivity expectations. Obtains medical record information for the referral.
Develops strong relationships with case managers, physicians, etc. at facilities. Works with Professional Relations Representatives (PRRs) to provide education on all available services. Provides care for any GIP patients in a hospital including daily visits, professional management, documentation and efficient discharge. Job Responsibilities: Makes after-hours visits to patients in their home, assisted living facility, nursing home, or hospital, anywhere within the assigned affiliate(s) territory. Provides compassionate customer service to patients and families backsses patient/family and identifies immediate needs and appropriate nursing interventions by phone and/or during visit.
Provides and manages direct care to patients and families as part of Interdisciplinary Group (IDG), incorporating psychosocial, spiritual, cultural, physical and biological components, and appropriate nursing intervention and follow-up. Demonstrates clinical skills regarding case management, application of company policy and procedure, physical backssment, intravenous therapies, pain medication titration, symptom management, and patient/family relationships. Prioritizes, consults, refers, and collaborates with other professionals to promote quality and continuity of patient/family care.
Educates patient, family, caregivers and other health professionals about disease process and decline, palliative interventions, care giving, dying process, and safety practices. Reports changes in the patient's condition to appropriate members of the IDG or other health professionals. Attends patient deaths; prepares body for transport; oversees disposal of medications; and provides bereavement support. Completes documentation per policy and procedure. Perform other duties as assigned.
