CHARACTERISTIC DUTIES AND RESPONSIBLITIES OF JOB: ESSENTIAL ELEMENTS: Assist in the coordination of initial pre-placement plans of consumers into the admission of the program, including gathering of intake information, interviewing consumer's caretakers, explaining the program, and participating in admission committee meetings. Cooperate with appropriate personnel in the evaluation of participants' behavior and in the backssment of the need and scope of intervention. Coordinate, develop, and review the implementation of consumers'
annual backssments and Individual Support Plans (including specific goals and projected goals), up to a maximum caseload of 30 program participants.
Submit accurate utilized CPS units weekly. Monitor and assist direct service staff to insure that Annual Plans and any approved behavior plans are implemented properly. Develop and evaluate current behavior support programs and present recommendations to the Interdisciplinary Team as necessary. Assure that county, state, federal, and agency policies on behavior support/restraint are complied with. Oversee Direct Support Professional's maintenance of consumers' daily service notes. Implement consumer behavior support plans.
Review and prepare progress notes on participants' behavior plans on an as-needed basis. Ensure that program activities are age and functionally appropriate. Attend all Individual Support Plan meetings when necessary. Maintain schedule of consumers' Individual Support Plan and required services, and advise the appropriate persons of the due dates. Assist the Interdisciplinary Team in the implementation of planned program activities to promote a consumers' social and functional growth and development. Consult with the Manager of Adult Day Services, Program Site Supervisor, Behavior Specialist, and the Interdisciplinary Team in the planning and implementation of behavior support plans.
Instruct direct service staff in the proper implementation of any approved behavior support plan. Maintain accurate and current individual consumer files/records and related meetings, counseling sessions, correspondence, etc. Maintain a record of individual consumer illnesses, traumas, injuries and seizures that occur at the facility or under supervision of the facility. Enter Unusual Incidents into EIM/HCSIS when required. Work with the Manager of Adult Day Services, Program Site Supervisor, Behavior Specialist, and the Interdisciplinary Team to continually monitor consumer behavior, appropriate program placement, and need for additional services.
