POSITION SUMMARY: Carries a caseload of high risk/ high utilizer patients and interacts with the Physician, patients and family as needed. Serves as a resource for the care team through support, education, and clinical expertise. Provides a network wide focus to the coordination and management of patient care. In collaboration with the other professionals assist with development and implementation of the patient's plan of
care. EDUCATION, TRAINING, EXPERIENCE, CERTIFICATION AND LICENSURE: Candidate will be a NYS Registered Nurse. Minimum of bachelor's degree in nursing or other heath related field with one to two years progressive experience in the field, master's degree preferred.
Previous discharge planning, home care, and/or managed care experience desired. Participates in orientation, education conference, updates and maintains knowledge and skills related to specific areas of practice. SPECIAL EQUIPMENT, SKILLS OR OTHER REQUIREMENTS: Organizational, management and leadership skills. Competent in BLS/CPR rescues. Excellent oral and written communication skills. Demonstrated ability for problem solving
and conflict management. Must be able to work both independently and as part of a group.
WORK ENVIRONMENT AND HAZARDS: OSHA category I. Routine or potential exposure to blood, body fluids, excretion or secretions. Work environment may be remote, hybrid or on site. PHYSICAL DEMANDS: Moderate work: lifting, bending, stooping, pushing and pulling. Heavy work: standing and walking. Light work: standing, walking, and lifting. Identify gaps in care and collaborate with patient/ family and other care team members to close them backss and support patients at each transition in care. Identify patients at high-risk or with high utilization patterns and collaborate with care team to eliminate barriers to efficient care delivery in the appropriate setting. Actively participate in performance improvement activities.
Complete patient backssment, at baseline and on-going and work with patient/ family to develop an appropriate plan of care. Educate patient/ family as needed.
