Serving families with one or more children (up to age 18), who are diagnosed as having a serious emotional disturbance (SED) and families having multiple service needs. Services are youth and family-centered and build on the youth

and family strengths to increase personal and community supports, promote healthy family functioning, support, and preserve families, and reduce the need for out-of-home placement.

Services are provided in-home/community through flexible scheduling. Per Diem positions are positions based on job duties assigned such as providing individual therapy. In this position you will be compensated for direct therapy services and administrative duties required under this treatment model. Direct therapy service is any time spent with the individual served that is considered billable under HPCT code definitions. Administrative time is any time spent for travel, meetings, supervision, documentation,

and trainings ESSENTIAL DUTIES AND RESPONSIBILITIES: Responsibilities associated with this job will change from time to time in accordance with the agency's program and business needs.

Offer a variety of clinical interventions including best practice and evidence-based models. Development of Individualized Safety/Crisis Plans with Families. Provide services during after-school and evening hours as well as possible weekend appointments when requested. Provide accurate monitoring and documentation of all case related activities to fulfill program obligations and requirements. Pursue broadened professional development to enhance level of competency and meet training requirements of contract.
