recruitment of schools for the Asthma Schools Recognition Program for grant years 2019-2024.

Conduct outreach in schools in Orange, Osceola, Seminole, Volusia, Sumter, Lake, Polk, Brevard and Indian River counties to encourage and assist the schools applying for the Asthma Friendly School Recognition Program. Provide asthma education and information to school nurses, school staff and students. Mentor all participating Asthma Friendly Schools in Orange, Osceola, Seminole, Volusia, Sumter, Lake, Polk, Brevard and Indian River counties. Travel to conduct outreach asthma friendly activities. Travel to facilitate the Open Airways for Schools (OAS) in participating Asthma Friendly Schools.

Coordinator will purchase and maintain educational materials and supplies needed for the support and the promotion of the Asthma-Friendly Schools Recognition Program. Deliverables include for Asthma Friendly in-home Visiting Program are as follow: This program is responsible for delivering preventive in-home asthma intervention services to clients in accordance with the Asthma Home Visiting Protocols and the EXHALE guidelines. Coordinator will: Execute a dated letter of agreement or memorandum of understanding to local referring partners. Establish additional agreements with health systems and health plans for referrals of individuals eligible for the home visiting program.

Schedule and deliver preventive in-home asthma intervention services to clients in accordance with the Asthma Home Visiting protocols and guidelines. Travel to in-home asthma interventions. Conduct outreach to increase referrals to free tobacco cessation service including Smoke-Free Multifamily Certified Communities and asthma in-home visits. Utilized the home visiting database to track services provided and outcomes from clients receiving home visits. All data will be backed up on the CHD’s network at least weekly to ensure no data loss.

All data will be subject to and must be compliant with the Health Insurance Portability Accountability Act of 1996 (HIPPA). Coordinator will follow the EXHALE technical guidelines in all strategies designed to expand and access, referral to delivery of coordinated community services, delivery of AS-ME, guidelines-based medication management to identify high burden asthma areas. Coordinator will purchase and maintain educational materials and supplies needed to support the operation of the Asthma Home Visiting Program. Have a designated office space with cabinets for confidential documentation storage.

Deliverables include for Florida Asthma Program are as follow: Coordinator will: Provide monthly updates to Asthma Program Manager and Asthma Program Evaluator. Enter data into the asthma home visiting tracking database for data collection throughout the entire project funding period. The Coordinator shall enter home visiting service data into the database within ten (10) calendar days of service delivery. Coordinator will provide from the database to the Florida Asthma Program Manager on a quarterly basis. (First quarter starting from September through December), by the 30th on the last month of the quarter.

Provide support for implementation of quality improvement project in the HQHC’s and Rural Health Centers; targeting geographic areas serving children with poorly controlled asthma. Attend all scheduled monthly Centers for Disease Control and Prevention (CDC) conference calls. Attend all Florida Asthma Program (FAP), Florida Asthma Coalition monthly meeting. Serve as Chair-woman of the Florida Asthma Coalition’s Schools and Child-Care workgroup. Serve as a core member of the Florida Asthma Coalition’s Home Visiting Program workgroup.

Attend or designate staff to attend all Florida Asthma Coalition Home Visiting Workgroup conference call to share program progress, lessons and best practice strategies. Attend additional conferences, summits and trainings as identified by the Program Manager. Promote and recruit AS-ME facilitators to increase the opportunities for community education. Conduct outreach to increase referrals to free tobacco cessation service including Smoke-Free Multifamily Certified Communities and asthma in-home visits. Develop and promote new Asthma-Friendly recognition opportunities such as Asthma-Friendly shop, Emergency department, childcare centers, hospitals and providers.

Disseminate marketing and awareness materials as provided by the Program Manager. Distribute air quality flags to existing and new Asthma-Friendly program participants. Promote and recruit AS-ME facilitators to increase the opportunities for community education. Promote the Community Health Worker model to reduce the triggers and burden of Asthma in Florida. Work in conjunction with other Florida Asthma Coordinators in Gadsden, Seminole and Miami-Dade Counties to promote Asthma Friendly Preventive Care. DOH Duties are as follow: Submit travel expenditures into FLAIR (Works) in accordance with allocation and CHD travel policies, but no later than 30 days the date of travel.
