Observe/report changes in the participant's condition to Home Health Coordinator of Manager Participate in training and in-service education programs. Complete other tasks as assigned by supervisor. Working Conditions : Perform participant transfers regularly with the use of mechanical devices as indicated. Travel to participants; homes and v isit multiple participants on the same day
(4-5 participants) Work with participants with varying cognitive levels, physical and sensory impairments, limited social supports, declining health and at end-of-life.
