Care is coordinated between CBHC programs and staff and other adult programs at The Brien Center (SUD services, Medication services, ACCS) and any external providers or state agencies. Long-term support is offered to appropriate clients and determined through on-going Utilization Reviews.

Essential Job Functions Provide support, outreach and engagement efforts to adults enrolled in CBHC. Help to identify cultural and linguistic needs and preferences and deliver person-centered care based on those needs. Continuously identify and resolve barriers to meeting goals complying with the Person-Centered Treatment Plan Help to facilitate therapeutic groups, as needed, to serve individuals. When

appropriate provide long-term support to individuals who have completed an episode of treatment with a clinician but requires on-going support In collaboration with the treatment team, develop and implement a treatment plan.

Evaluate progress towards treatment plan goals and modify as needed. Facilitate referrals to care, and other supports to ensure that clients attend appointments. Assist with transportation to needed appointments as necessary. Assist individuals in navigating the network of community-based services and information. Support safe transitions of care for individuals moving between care settings and services. Facilitate communication between individual or designated representative

and medical, behavioral, and social service providers.

Attend meetings as required or requested. Maintain client records, completing all required documentation in a timely and accurate manner. Participate in trainings as required. Other Responsibilities Participate in regularly scheduled supervision Attend required agency trainings Qualifications BA/BS in relevant Human Services field required Experience in community-based behavioral health support programs preferred. Experience with accessing local resources, navigating health, behavioral health and/or substance use treatment systems preferred. Valid driver’s license with reliable transportation and appropriately insured vehicle required.
