PRINCIPAL DUTIES AND RESPONSIBILITIES: The Nurse Navigator will: 1. Be the first point of contact for newly referred patients and will make sure the patient's chart has all the necessary medical information. a) Track down records from outside hospitals/doctors (including imaging docs) b) Perform chart review/data extraction

Dates and results of pertinent labs d) Welcome each new patient to our Center by explaining the services we offer and presenting a welcome packet to each new patient e) Give out and explains the purpose of the PROMS survey. He/she will answer any questions that the patient has. 2. Coordinate referrals and decide which services will be appropriate for each patient. a) Go through

referrals and determine which ones are appropriate for the Crohn's and Colitis Center b) Determine which patients should participate in multidisciplinary clinic vs regular clinic after thoroughly looking thoroughly looking through medical history c) Answer dedicated phone line for patients and providers who want to refer patients to the Center d) Will be physically present during multidisciplinary clinic and will help patient with coordination of services e) May be asked to help with patient paperwork/insurance related work 3.

Provide competent, compassionate patient centered nursing care based on scientific principles and the use of the nursing process and interdisciplinary teamwork.
