a comprehensive rehab care continuum at Uof L Health - Frazier Rehab. The Rehab Nurse Navigator is an integral part of the rehab team for patients prior to inpatient admissions and beyond outpatient rehab discharge.

The Rehab Nurse Navigator provides patient care, education, documentation, and facilitates communication amongst patients, support systems, Uof L Health team members, and physicians. The Rehab Nurse Navigator tracks performance improvement measures for specialty populations including patient experience initiatives and quality care metrics and demonstrates a commitment to improving the rehab services provided. A focus of the Rehab Nurse Navigator is to promote patient and staff

education by providing group education opportunities and competency training sessions.

A Rehab Nurse Navigator will facilitate the development, coordination, and implementation of a patient’s plan of care. It is the responsibility of the Rehab Nurse Navigator to promote rehab staff development and competency completion in the delivery of care to patients with specific diagnoses requiring rehabilitation and serves as a key liaison with clinical educators. The Rehab Nurse Navigator ensures appropriate equipment, supplies, and proper training is available to support the clinical team and the specialty population.
