Responsibilities: The Virtual Registered Nurse (Virtual RN) works onsite in our Silver Spring location in the Virtual RN Hub. The Virtual RN is a member of the Nursing Care Team and is responsible for collaboration, communication & facilitation and documentation of optimal provision of care through a computerized and/or digital platform supporting daily management of care. Responsible for the backssment, planning, implementation, coordination, monitoring & evaluation of the patient's plan of care from admission to discharge. Organize care through collaboration with multidisciplinary care team and participates directly or indirectly with patient Utilizes clinical knowledge and critical thinking skills coordinate, implement & support the plan of care.
Ensures a seamless, effective, efficient transition of care across the continuum. Responsible for monitoring quality metrics specific to the department & ensuring complete & accurate documentation in the patient record. Performs virtual Safety Rounds A high degree of professional competence and autonomy will be demonstrated in performing various roles such as clinician, educator, consultant/collaborator, and navigator.
What you will need Required: Baccalaureate Degree in the Science of Nursing (BSN) degree from an accredited school of nursing. Current licensure by the Maryland State Board of Nursing or Compact State as a Registered Nurse. Minimum eight (8) years of experience in health condition management, which must include adult condition management. Electronic Medical Record (EMR) experience.
