What You’ll Do: Assist Healthcare professionals in delivering patient care Monitor, records, and communicate patient condition as appropriate to care team, physician, patient, and family Collaborate as needed across disciplines to coordinate patient care, including patient transfer, discharge, referral, and spiritual/psychosocial support needs Evaluate learning needs of patient and/or family and provide patient/family
