Stephens Indian School. Job Details: Job Title: Physical Therapist Location: St. Stephens Indian School which is located at 128 Mission Rd. St. Stephens, WY 82524. Schedule: Part-Time. Requirements & Skills: Must be a Doctor of Physical Therapy (DPT) degree Must have at least 5 years of experience, preferably with Native

American students Must be certified by American Board of Physical Therapy Specialties (ABPTS) Must be Licensed in the state of Wyoming Tasks, Duties & Responsibilities: Provide all necessary supplies, materials, equipment and testing materials to perform physical therapy services.

To include: Screenings and formal evaluations Training for teacher, when requested Physical therapy evaluations for all students referred for evaluation Participation during the initial placement or multidisciplinary team committee meeting for students evaluated by the physical therapist Advisement to the special education and classroom teacher in matters regarding continuous backssment, evaluation and implementation

of motor therapy programs for individual students and will meet with individual students on a regular basis to provide suggestions for carryover of therapy strategies into home when necessary Specific Duties & Responsibilities: After being provided a copy of parental permission form for backssment by the school, the Physical Therapist shall: Provide diagnostic services and reports for students that are identified with a disability and are in need of physical therapy services Provide consultation to school staff regarding accommodations and modifications needed for students eligible for physical therapy services Assist the school in consulting with or obtaining services from medical providers Participate in Individualized Education Program (IEP) and in multidisciplinary meetings to review evaluation results, integrate findings with other disciplines, offer recommendations and develop individual education plans and intervention plans to achieve IEP goals.

Participate in multidisciplinary meetings as required Provide training to school personnel as requested Responsible for the maintenance of equipment such as wheelchairs, crutches and adaptive equipment Provides school physical therapy as mandated on IEP Provide training for parents as required Consults with other service providers to develop cohesive service plan; may provide cotreatment Meet with the parents of students and/or guardian of students placed for therapy purposes, to provide for consultation during which time the contractor will be available to meet with close family members to discuss those issues, problems or concerns which are pertinent to or impact on the progress of the student within the school environment.

DESCRIPTION OF ACTIVITIES: Forklift operators perform a variety of job functions. These functions include, put away, pulling

and restocking, truck unloading and servicing the retail will-call desk. The put away driver’s main responsibility is to put away product that has been received.

The “Gladiator” RF scanner will be used for this process. The pulling and restocking driver’s main responsibility is to pull the product identified in the restocking reports and putting the full pallets into the staging areas. The productivity level for put away is 100 cartons per hour and the productivity level for pulling is 60 per hour. Forklift operators will be trained and certified by the forklift certifier prior to operation and are recertified annually. Forklift drivers will perform any of these job functions as workload

demands. LEVEL OF PHYSICAL EXERTION: Level 4 - Continuous walking or standing, pushing or pulling, regular lifting: weights up to 50 pounds.

(Safety harness must be worn at all times when operating the lifts) SKILLS REQUIRED: Physically be able to lift up to 50 pounds regularly and work on your feet all day. Must be able to operate the lifts properly and safely. Cannot have a fear of heights. Must pay attention to details and have a valid Driver’s License. Forklift experience is required.
