The Social Worker is an integral member of the multi-disciplinary treatment team. The Social Worker participates in interdisciplinary treatment planning; completes psychosocial evaluations of admitted patients and meets the documentation and turn-around-time standards.
The Social Worker facilitates the discharge planning process on day one of treatment and maintains communication with collateral resources. The Inpatient Social Worker maintains patient charts appropriately, including accurate documentation of backssments, coordination of care efforts, discharge planning efforts and other interventions throughout the patient's episode
of care. The ideal candidate will be a team player, have excellent organizational skills, effective communication skills and the ability to work well within a multidisciplinary disciplined environment, maintaining positive relationships with physicians, patients, families and all other hospital employees.
Education: Broad knowledge of social work practice, Master's Degree in social work, Mental Health Counseling or Psychology from accredited programs, RI Board Certified Alcohol and Drug Profession Licensure: Master's Prepared or LCSW/LICSW/LMHC/LCDP Experience: One - two years of clinical work in acute care settings recommended.
