THE ADVOCATE HOME HEALTH NURSE: Assumes primary responsibility for the backssment, planning, implementation, and evaluation of nursing care for assigned patients in the comfort of their own homes. Nurse/Patient Relationship: Develops and maintains a therapeutic relationship throughout the healthcare continuum.
