The Maternal Infant Coach will facilitate access to comprehensive services that include, but are not limited to nutritional counseling, food assistance, oral health care, mental health/wellness services, substance abuse, prevention and/or treatment, and emergency shelter/housing. The Maternal Infant Coach will engage enrolled pregnant women and their families in discussions about program options, plan for the infant's transition to program enrollment, and provide support to the family during the transition process, where appropriate. Newborn/Infant: Provide a newborn visit within two weeks after the infant's birth to offer support and identify family needs.
