Works with RN Manager to develop QI processes for successful care management integration. Coordinates monthly collaborative Care Rounds. Develops and implements QI care plan summary templates for KP EHR. Develops and implements a tool box for initial HFA and reassessments for CM/CC. Essential Responsibilities: Develops the QI care management tool box for initial and reassessment materials needed for a home visit. Develops QI check lists and implements

them. Receives and performs initial triage of paraprofessional. Assists with resource development. Networks with other community based services. Works closely with CM/CC for referral regarding Home Community Based Services (HCBS)/Durable Medical Equipment (DME), dental services, Social, Emotional, and Behavioral Difficulties (SEBD)/Severely Mentally Ill (SMI), Child Welfare Services, Adult Protection Services (APS), and Developmental Delayed (DD).

Provides member care services to Kaiser Permanente QI enrollees. Navigates services based on medical, social and psychosocial needs, educates members on QI program and offers assistance as needed. Navigates transportation and housing needs for

members and/or their families for medical services both Inter island and on the Mainland.

Works with RN manager to assign QI member caseloads to RN CM and LSW CC. Develops and implements a HFA tracking form for RN CM/LSW CC in home visits and follow-ups. Interprets and explains services of Kaiser Permanente QI program to members and families. Acts as QI member and family advocate. Navigates the health care system and health care services available to our members. Acts as liaison to promote Kaiser philosophy and services. Confirms QI coverage to patients and service providers within Kaiser as needed. Basic Qualifications: Experience Three (3) years community outreach in healthcare setting, or related experience.

Education High school diploma or GED. Primary purpose of the role: We are currently looking for a Mitigation Technician to join our team. Once our Customer Care team receives a call regarding property damage, our Mitigation Techs are the first to arrive on site. They are quick to act, backss the damage that’s been done and work with the customer to help stabilize the damage. This role is incredibly challenging as not all buildings are built the same and not all

damage affects property the same. We want someone who is analytical, who is naturally curious, and who can act quickly. Our techs work in teams and collaborate all day as there is quite a bit of coordination that goes into stabilizing damage and helping customers understand next steps in the restoration process.

What you’ll be doing each day: Daily response to possibly multiple customers who have water, mold, fire, and wind damaged properties both during and after business hours Drive company vehicle to job sites backss damage and work with Mitigation Lead to develop action plan based on each customer’s unique situation Deploy and operate appropriate equipment Utilize industry specific

equipment to dry the structure successfully to industry standards Perform demolition of affected areas and removal of appliances Work with customers who wish to store their personal items in our warehouses to include inventory of items, assembly of storage containers, and relocation of property into storage containers.

Act in a highly sensitive, positive, and professional manner with all clients and team members Document all progress in job management software Periodically communicate job updates to customers Self-direct your industry learning and training What you’ll need in order to succeed: Proof of a clean driving record The ability to lift up to 50 lbs, and move large equipment in and out of trucks and homes/buildings The ability to stand and work for prolonged periods thru-out the day The ability to bend at the knees and waist in a safe manner Past work performance in a repair, maintenance or construction field is desirable Ability to show good communication skills, written and spoken Ability to show proficient use of technology, computers, software, systems, and mobile devices.
