This part time role will float between Children's Hospital, Emergency Department, and Adult Medicine and will be a critical member of a multidisciplinary care team dedicated to providing the best and most advanced care and support. This is a wonderful opportunity for an experienced social worker to expand their skills in a hospital setting with multiple patient populations.
Our exemplary team of Inpatient Social Workers are essential liaisons between patient, family, physicians, community and organizational resources to promote the coordination of a comprehensive treatment plan. Our Inpatient Social Workers collaborates with physicians, staff nurses, and care coordinators to determine
patient care needs, readiness for discharge, and the need for home-health services or other community-based resources. This individual will play a significant role in making connections and referrals to the health agencies, mental health facilities, counseling services, and other social and governmental departments that would be the best resource in ensuring patient needs are met.

Masters of Social Work degree from a Council of Social Work Education school of social work with 1 to 2 years effective experience required. Minimum of two
years of acute care case management/social work experience. Knowledge of PC based computer software (Cerner, Navihealth/Curaspan).
Current social work licensure in Pennsylvania.
