Other duties as assigned. Qualifications Basic Qualifications & Experience High School Diploma or equivalent required. Associates degree or equivalent certification preferred. Minimum of 1-year experience working with senior adults in a dementia social or recreational programming required. Summary of Position: This position delivers health care to detained or committed high risk adolescents in a locked, secure setting in accordance with the National Commission on Correctional Healthcare (NCCHC) standards and other nationally recognized standards. This position provides primary care with responsibility for a continuum of care from the initial backssment through development and maintenance of health care including physical, psychiatric, and dental maintenance.

Clinic hours for this position are day/afternoon/evening depending on the shift offered. We have multiple openings. Specific Job Duties: Clinical Duties: Ensures the provision of patient care is within standards established by the State Nurse Practice Act and the Division of Youth Services medical policies, procedures, and protocols. Provides initial nursing backssments and triage of requests for health care submitted by youth.

Works in collaboration with medical providers to obtain laboratory and other diagnostic procedures. Administers nursing care and provides the youth education for routine, urgent and emergent medical situations within scope of practice. Makes decisions regarding infectious/communicable diseases, recommends referrals for specialty interventions for identified conditions and recommends supervisory review of complex medical and mental health issues in collaboration with providers and behavioral health specialists. Addresses and communicates confidential information regarding medical, dental, psychiatric, and mental health care to non-medical personnel with appropriate information to maintain the resident in a safe and healthy environment in accordance with Health Insurance Portability and Accountability Act (HIPAA) regulations.

Documents all care in the electronic health record. Mental Health: Evaluates resident's mental status/mental health condition through interview, health screen, health appraisal and observation. Provides basic mental health intervention and refers to psychiatrist or mental health resource for further evaluation, intervention or transfer to a mental health institution in collaboration with behavioral health services staff.

Conducts mental health referrals as necessary by coordinating with outside agencies/courts for appointments or furloughs. Actively involves parents and legal guardians in the care and treatment of the mental health of their youth. Medications: Follows national standards of care for safe medication administration within the state Nurse Practice Act and the Division of Youth Services policies, procedures and protocols. Transcribes, orders, prepares, and administers medications without error per prescribed provider's orders.

Documents, updates Medication Administration Record (MAR) by entering information into electronic health records. Monitors MARs for errors and reports errors to medical clinic supervisor and facility administration. Provides necessary education or training to staff on medications as needed. Checks in medications and packages medication for youth leaving the facility. Maintains clinic environment by ordering and maintaining stock supplies and equipment for the clinic and medical supplies throughout the facility. Health Education: backsses and identifies the knowledge deficits regarding general health and hygiene topics presented by youth in routine and formal encounters. Provides individual and group education regarding health promotion, disease and injury prevention to youth and staff on an ongoing basis. Provides ongoing education regarding medication administration, infection prevention and other health related topics specific to staff.
