Secures equipment and supplies required based on plan of care, including medicals for Hospice patients. Evaluates own needs and seeks personal support and/or guidance of management or another member of the interdisciplinary team, as needed. Records in a timely manner evaluation data, the plan of care, interventions, client's response to interventions, and all other clinical patient documentation in the electronic medical record.
Participates in quality/performance improvement measures. Employees with e-mail are required to maintain proficiency in the basic functions of the program and are also required to regularly check email and keep calendars up to date.
