In addition, call all recent inquiries on a weekly basis to sell our services and obtain an in-home backssment appointment. Expectation: To ensure at least 50% of the leads we receive close. To ensure that 90% of the home backssments close. 2. Visit clients’ homes and discuss all aspects of Visiting Angel's services. All initial visits will focus on the client's needs, what the client likes to do, and the type of caregiver the client would enjoy having with them. 3. Create a client file for all cases with signed contracts.
Make sure the client file is current at all times. Collect and maintain all client paperwork, and also enter all client information into the tracking software. Ensure that the pricing of a case is within the stated policies of the company. This will ensure the company will maintain its profit levels on a case-by-case basis. 4. Maintain a list of all new clients and track conversion rates, calls and sources. Track all closed cases. Complete all follow-up procedures as outlined in the company’s policies. Help the Staffing Coordinator confirm new client assignments, including the introduction of the client to the caregiver.
5. Works closely with the nursing staff to ensure initial backssments are scheduled. Prepares binder notebooks for the client’s home with directions and forms for the caregiver. Consult with the Staffing Manager to ensure that the proper level of care is being delivered. Visits current clients in the hospital/rehab/nursing home to check up on them and maybe take them a gift. 6. When service has ended for a client, prepare discharge paperwork including a letter to the family with the caregiver evaluation survey. Periodically updates the “Client Emergency Planning Forms” notebook.
Attends weekly staff meetings and reports on hours serviced and conversion rates. 7. Attend symposiums and expos to promote our services, including ordering/suggesting promotional items. Thoracic, Cardiac, and Vascular (TCV) surgical patient population. TCV surgery patients are admitted for procedures including coronary artery bypass graft (CABG), cardiac valve repair and replacement, AAA repair, lung resection, esophagectomy, heart and lung transplant, limb amputations, femoral popliteal bypass, and carotid endarterectomy.
The nurse-patient ratio is 4 or 5:1 in acute care and 3:1 in TIMU. 4 West is 30-bed unit which includes 18 acute-care beds and a 12-bed TCV Intermediate Care Unit (TIMU). The TIMU provides care for TCV patients who do not need the intense ICU environment of our CVICU (formerly TCV Post Op unit, ) yet need more advanced monitoring than an acute-care
bed. TIMU patients include, but are not limited to, fresh lobectomies, patients receiving the thrombolytic TPA, AAA, CABG/Valve and Lung transplants. Some patients are direct admits from PACU, while other come from the CVICU (formerly TCVICU) unit.
