What You Will Do: The RN Case Manager works collaboratively with patients, social workers, physicians, other practitioners, caregivers and
community agencies in coordinating and overseeing the interdisciplinary plan of care for a patient in a home care/home hospice environment.

Ideal candidates also possess negotiation skills along with strong analytical and computer skills.
Experience with pre-acute and post-acute care as well as community resources is preferred. Case Managers also promote safety, cost efficiency and a commitment to quality improvement. What you will need: Associate’s Degree in Nursing required. Bachelor's Degree in Nursing preferred. Current State of WI or e NLC compact state RN license. Valid State of WI driver’s license with an acceptable driving record. Current BLS (CPR) certification or ability to obtain upon hire. Clinical experience preferred. WHAT YOU’LL BRING: Ability to successfully achieve criteria of Trainee Program. Must pass and apply for appropriate insurance licenses within 14 days of entry into the job, max of 2 attempts at Life and Health exam. Highly self-motivated and results-oriented with ability to work independently and as part of a team. Access to reliable transportation in order to attend appointments and meetings. Ability to travel up to 50% of the work period and maintain a valid driver’s license.
