You work closely with patients, family members, social workers and caregivers

to: - Provide direct nursing care to patients with select disorders of the gastrointestinal, genitourinary and integumentary systems in accordance with established policies, procedures and protocols of the healthcare organization.

Implements and monitors patient care plans. - Monitor, record and communicate patient condition as appropriate. - Provide patient and family education on common topics including wound care, dressing applications, compression therapy, diabetic foot care, venous disease, wound offloading, stoma care, pouching procedures and underlying disease processes. - backsses and coordinate patient's discharge planning needs with members of the healthcare team. Job Responsibilities: Performs comprehensive nursing backssment for those patients within scope of practice. Collaborates with the physician and multi-disciplinary care team to coordinate ongoing patient care. Uses data to develop and implement nursing interventions as appropriate to meet patient care needs and facilitate the discharge process.

Performs wound/ostomy/continence/skin care based on sound nursing theory, scientific principles, evidence-based practice, professional judgment, and individual patient needs. Follows patient along continuum of care. backsses and triages patients appropriately. Within scope of practice, monitors and documents patient's condition, effectiveness and timeliness of care and/or interventions; evaluates, updates and revises plan of care to facilitate achievement of planned and expected outcomes. Delegates appropriate components of the nursing care plan to other members of the nursing team backss educational needs of the patient and family/significant other considering cultural issues, age appropriate and other special concerns.

Provides teaching and educational material to patient and/or family members/significant others concerning subject matter as appropriate for the specific patient need which may include diagnosis, safe and proper use of equipment, medications, self-care, or nutritional needs. Validates and documents patient and family understanding of teaching. Designs, directs, implements and evaluates educational offerings based on knowledge, skills and learning levels of nursing staff for wound/ostomy/continence/skin patient care.

Works with the Nursing Institute to identify learning needs of nursing personnel. Others duties as assigned Education: Graduate from an accredited school of professional nursing. BSN required. Certifications: Current licensure as an RN required24 months to obtain wound ostomy continence certifications through the Wound Ostomy Continence Certification Board (WOCNCB)Current Basic Life Support (BLS) through American Heart Association (AHA) certification required Competencies (Complexity of Work): Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.

Must be able to work in a stressful environment and take appropriate action. Work Experience: 1 year recent RN experience required2 years of recent RN experience preferred Physical Requirements: Light Work - Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly (Constantly: activity or condition exists 2/3 or more of the time) to move objects. Even though the weight lifted may be only a negligible amount, a job should be rated Light Work: (1) when it requires walking or standing to a significant degree; or (2) when it requires sitting most of the time but entails pushing and/or pulling of arm or leg controls; and/or (3) when the job requires working at a production rate pace entailing the constant pushing and/or pulling of materials even though the weight of those materials is negligible.

Note: The constant stress and strain of maintaining a production rate pace, especially in an industrial setting, can be and is physically demanding of a worker even though the amount of force exerted is negligible.

Responsibilities: Provides professional nursing care to patients undergoing diagnostic or therapeutic services to hospitalized patients in a variety of healthcare settings backsses patient; completes nursing backssment, documents findings in patient records and notifies physician of abnormal findings. Establishes patient care priorities and delegates nursing activities according to the patient needs and staff qualifications and capabilities.

Serves as charge nurse when assigned to ensure efficient operations and continuity of care. Participates in education, research and performance improvement activities. Provides health education to patients and families. Develops, evaluates, adapts and documents education to meet learning needs and abilities of patients and families. Assists in the preparation of department specific education for patients and colleagues. Covers other nursing roles in the department. Maintains core competency in nursing (CPR, skills labs). Other duties as assigned. Education: Graduate of an Accredited School of Nursing.

BSN preferred. Certifications: Current state licensure as a Registered Nurse (RN). Basic Life Support (BLS) through American Heart Association (AHA) required. In areas performing moderate sedation procedures must have current Advanced Cardiac Life Support (ACLS) through American Heart Association (AHA) Certification within 6 months of hire. If in an ED, Pediatric Advanced Life Support (PALS) through American Heart Association (AHA) upon hire or within one year of beginning work in the Emergency Department. In areas operating with Peripherally Inserted Central Catheters (PICC Lines), Certified Registered Nurse Infusion (CRNI) or Vascular Access Board Certified (VA-BC) certification is preferred within three years of hire.

Complexity of Work: Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action. Work Experience: Minimum of one year recent RN experience required; two years of recent RN experience preferred For Cardiac Catheterization or other related procedural areas, one year of critical care experience in a hospital setting preferred.

Current demonstrated clinical competence. Strong communication and interpersonal skills. Physical Requirements: Requires full range of motion, manual and finger dexterity and eye-hand coordination. Requires standing and walking for extensive periods of time. Requires corrected hearing and vision to normal range. Requires some exposure to communicable disease or bodily fluids. Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects. Physical Demand requirements are in excess of those for Light Work.
